Механизмы возникновения и агрессивности течения заболевания получают все большую аргументацию. Понимание биологии опухолевого роста вносит свои изменения в систему лечения пациентов с КРР. В основу представления о разнородности фенотипов опухоли были положены современные молекулярные и патоморфологические методы диагностики.</p></abstract><trans-abstract xml:lang="en"><p>In last years, the attention of researchers is focused on the study of carcinogenesis and differentiation tumors of colorectal cancer (CRC). Mechanisms of the onset and aggressiveness of the disease are gaining more and more arguments. The biology of tumor becomes more understandable and introduces its changes into the system of treatment of patients with CRC. The heterogeneity of tumor phenotypes was based on modern molecular and pathomorphological methods of diagnosis.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>колоректальный рак</kwd><kwd>tumor budding</kwd><kwd>метастазы в лимфатические узлы</kwd><kwd>прогностические факторы</kwd><kwd>патоморфологическое исследование</kwd><kwd>ITBCC</kwd></kwd-group><kwd-group xml:lang="en"><kwd>colorectal cancer</kwd><kwd>tumor budding</kwd><kwd>lymph node metastases</kwd><kwd>prognostic factors</kwd><kwd>pathological examination</kwd><kwd>ITBCC</kwd></kwd-group></article-meta></front><body><sec><title>ВВЕДЕНИЕ</title><p>Более чем 90 % случаев опухолей толстой кишки представлены аденогенным раком — аденокарциномой различной степени дифференцировки [<xref ref-type="bibr" rid="cit1">1</xref>]. Классификация TNM является международным стандартом для стратификации пациентов с КРР на группы риска. Тем не менее различные показатели выживаемости больных с идентичной стадией заболевания демонстрируют необходимость поиска дополнительных прогностических факторов.</p><p>В многочисленных международных исследованиях было показано, что на роль такого фактора претендует tumor budding (TB), являющийся независимым прогностическим фактором при колоректальном раке [2–6]. Значение TB подразумевает наличие одиночных опухолевых клеток или опухолевых кластеров, сформированных в группы от 2 до 4 опухолевых клеток, которые расположены отдельно от основной массы опухоли, главным образом в инвазивном фронте [6–8]. Впервые данное явление было описано при раке желудка в Японии в 1949 году [<xref ref-type="bibr" rid="cit9">9</xref>]. В 1950–1960-x годах японские исследователи обнаружили взаимосвязь TB с прогнозом при опухолях языка, гортани, молочной железы, желудка и шейки матки. TB при КРР был упомянут в 1990-х годах [<xref ref-type="bibr" rid="cit10">10</xref>][<xref ref-type="bibr" rid="cit11">11</xref>].</p><p>В литературе широко распространена гипотеза, что TB при КРР — это клетки на стадии эпителиально-мезенхимального перехода (ЭМП) [<xref ref-type="bibr" rid="cit16">16</xref>]. До 1980-х годов ЭМП был известен как процесс, связанный с эмбриогенезом и репарацией тканей. Эволюция эпителиальных клеток в мезенхимальные теоретически может играть роль в канцерогенезе и метастазировании [12-15]. Также существует гипотеза, что формирование TB является одним из первых этапов метастазирования.</p><p>До сих пор нет русского термина или адекватного перевода tumor budding. На III ежегодном Kонгрессе Российского общества онкопатологов на сессии по протоколу вырезки опухолей прямой кишки был предложен перевод термина как «степень почкования». Кроме того, используются термины «опухолевое почкование» или прямая транслитерация «баддинг» и «бадд».</p><p>В данном обзоре представлены понятия о влиянии TB на прогноз и дальнейшую тактику лечения КРР, а также методы его оценки, в том числе при помощи цифрового анализа изображений (ЦАИ).</p></sec><sec><title>МЕТОДЫ</title><p>Для написания данного обзора литературы мы выполнили поиск англоязычной литературы по ключевым словам c использованием следующих web-ресурсов: MED-LINE, Scopus, Web of Science, EMBASE и Google Scholar.</p><p>Поисковые запросы не были ограничены датой публикации и включали всю существующую литературу, опубликованную до ноября 2024 года. Публикации клинических случаев были исключены из списка анализируемой литературы.</p><p>Полученные данные были оценены независимо каждым автором.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>При анализе существующей литературы мы выделили следующие клинические аспекты TB (табл. 1).</p><table-wrap id="table-1"><caption><p>Таблица 1. Кроме стандартного патоморфологического заключения клиническому онкологу важно знать информацию о нарушении репарации белков (MLH1, MSH2, MSH6, PMS2) и молекулярно-генетическом статусе опухоли. В некоторых исследованиях установлено, что предоперационный биопсийный материал может иметь важную прогностическую ценность, основанную на оценке TB. В частности, степень баддинга в биопсийном материале может отражать вероятность наличия метастазов в регионарных лимфатических узлах, связь с ответом на неоадъювантную терапию и низкую выживаемость пациентов с раком прямой кишки [<xref ref-type="bibr" rid="cit35">35</xref>][<xref ref-type="bibr" rid="cit36">36</xref>]. Поскольку инвазивный фронт опухоли редко попадает в образец биопсийного материала, для прогностического значения оценивают внутриопухолевый TB [<xref ref-type="bibr" rid="cit37">37</xref>]. Кроме биопсийного материала клиницист должен ориентироваться на инструментальные методы диагностики, которые имеют важную прогностическую ценность для принятия клинического решения в отношении тактики ведения пациентов с КРР.</p></sec><sec><title>Ранние стадии КРР</title><p>Согласно AJCC (American Joint Committee on Cancer) и UICC (International Union Against Cancer) ранним КРР принято считать 0 и I стадии (pTis/1, N0, M0). Несмотря на то что большинству пациентов выполняют радикальное эндоскопическое удаление малигнизированных полипов, до 13,2 % уже имеют метастатическое поражение регионарных лимфатических узлов [<xref ref-type="bibr" rid="cit18">18</xref>]. Это диктует необходимость поиска дополнительных прогностических факторов для стратификации пациентов, потенциальных кандидатов для резекции толстой кишки.</p><p>Полагаясь на результаты ретроспективных когортных исследований, оценка TB может предоставить важную информацию о возможном метастазировании в регионарные лимфатические узлы. В 2013 году Beaton и др. [<xref ref-type="bibr" rid="cit19">19</xref>] выполнили метаанализ 23 когортных исследований, включавших 4 510 пациентов с опухолями pT1. В своей работе авторы показали, что при выявлении &gt; 10 опухолевых кластеров (Bd 3) TB имеет прямую взаимосвязь с наличием метастазов в регионарных лимфатических узлах (отношение шансов [ОШ], 7,74; 95 % доверительный интервал [ДИ], 4,47–13,39; р &lt; 0,001), превышающий относительный риск (ОР) других неблагоприятных прогностических факторов, в том числе больше, чем глубина инвазии в подслизистый слой, на 1 мм (ОШ, 3,87; 95 % ДИ, 1,50–10,00; р = 0,005), лимфатическая инвазия (ОШ, 4,81; 95 % ДИ, 3,14–7,37; р &lt; 0,00001) и низкая дифференцировка опухоли (ОШ, 5,60; 95 % ДИ, 2,90–10,82; р &lt; 0,00001). Данные выводы были подтверждены отдельными исследованиями из Японии [<xref ref-type="bibr" rid="cit20">20</xref>] и Нидерландов [<xref ref-type="bibr" rid="cit21">21</xref>].</p><p>В 2014 году Ueno и др. проанализировали 3 556 пациентов с опухолями КРР рT1 из 30 японских медицинских центров. Наиболее значимыми факторами риска метастазирования в регионарные лимфатические узлы являлись: степень TB, низкая дифференцировка опухоли, сосудистая инвазия и инвазия в подслизистый слой (р &lt; 0,0001, для всех факторов) [<xref ref-type="bibr" rid="cit20">20</xref>].</p><p>Метаанализ, состоящий из 17 исследований более чем с 3 500 пациентами, показал, что инвазия в лимфатические сосуды (ОР, 5,2; 95 % ДИ, 4,0–6,8), инвазия в подслизистый слой на 1 мм (ОР, 5,2; 95 % ДИ, 1,8–15,4), TB (ОР, 5,1; 95 % ДИ, 3,6–7,3), как и низкая дифференцировка опухоли (ОР, 4,8; 95 % ДИ, 3,3–6,9), являются значимыми факторами риска метастазирования в регионарные лимфатические узлы [<xref ref-type="bibr" rid="cit21">21</xref>].</p><p>В корейском исследовании 2022 года Jeong-ki Kim и др. установили, что низкая дифференцировка (ОР 8,16; 95 % ДИ 1,80–36,89), лимфатическая или венозная инвазия (ОШ 5,91; 95 % ДИ 2,91–11,97) и пять или более опухолевых кластеров (ОШ 3,01; 95 % ДИ 1,21–7,69) были независимыми факторами риска. При использовании этих трех факторов риска частота метастазирования в ЛУ составила 5,6, 15,5, 31,0 и 52,4 % при общем количестве баллов 0, 1, 2 и ≥ 3 соответственно.</p><p>Согласно рекомендациям NCCN (National Comprehensive Cancer Network), TB относится к дополнительным параметрам патоморфологического исследования малигнизированных полипов, а также одним из факторов риска рецидива при II стадии колоректального рака.</p></sec><sec><title>КРР II стадии</title><p>КРР II стадии (рТ3/Т4, N0, M0) охватывает гетерогенную группу пациентов с 5-летней общей выживаемостью 70,3 %, при подгрупповом анализе выживаемость для IIC стадии составляет 54,6 %. Эти цифры явно контрастируют с 5-летней общей выживаемостью пациентов с поражением лимфатических узлов: 58,3 % (в совокупности для всей III стадии) и 79 % (для III А стадии — метастатическое поражение 1 лимфатического узла) [<xref ref-type="bibr" rid="cit23">23</xref>]. Очевидно, что некоторые пациенты с II стадией КРР, у которых в исследованных лимфатических узлах не были обнаружены метастазы, будут иметь риск развития раннего прогрессирования после радикального удаления первичной опухоли.</p><p>Исходя из этого, выявление пациентов с II стадией заболевания и высоким риском прогрессирования с использованием дополнительных параметров патоморфологического исследования важно для принятия рационального клинического решения в отношении необходимости назначения адъювантной химиотерапии (АХТ). Назначение АХТ для пациентов, имеющих высокий риск прогрессирования, должно тщательно обсуждаться с учетом всех потенциальных рисков и перспектив [24–26]. Результаты многочисленных ретроспективных когортных исследований свидетельствуют о наличии взаимосвязи между TB и неудовлетворительными показателями общей выживаемости (ОВ) и безрецидивной выживаемости (БРВ) пациентов с КРР II стадии [27–33]. При ретроспективном анализе 446 больных КРР выживаемость при II стадии с выявленным TB Bd 3 принципиально не отличалась от III стадии [<xref ref-type="bibr" rid="cit34">34</xref>]. Кроме этого, наличие TB связано с патоморфологическими особенностями: лимфатической и венозной инвазией, высокой дифференцировкой опухоли и инвазией в край резекции [27–33].</p><p>Таким образом, TB является важным дополнительным прогностическим фактором КРР II стадии, позволяет выявить пациентов с высоким риском прогрессирования, потенциальных кандидатов для проведения АХТ.</p></sec><sec><title>Стандартизованная система оценки TB</title><p>По инициативе USCAP (United States and Сanadian Academy of Pathology) в апреле 2016 года в г. Берне, Швейцария, состоялся международный консенсус, организованный для стандартизации системы оценки TB [<xref ref-type="bibr" rid="cit38">38</xref>]. В ходе 7 сессий были сформулированы следующие постулаты.</p><p>1. TB — это одиночные малигнизированные клетки или опухолевый кластер, состоящий не более чем из 4 клеток.</p><p>Исходные данные основаны на исследованиях оценки TB как одиночных опухолевых клеток или кластера опухолевых клеток, состоящего не более чем из 4 клеток [<xref ref-type="bibr" rid="cit2">2</xref>][<xref ref-type="bibr" rid="cit4">4</xref>]. Прогностическая ценность TB не зависит от количества клеток, формирующих кластер [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit6">6</xref>][<xref ref-type="bibr" rid="cit9">9</xref>]. Наиболее часто при оценке TB на срезах встречаются кластеры, состоящие из 4 опухолевых клеток. Кластеры, состоящие из 5 и более опухолевых клеток, называют низкодифференцированным кластером (PDC — poorly differentiated cluster) [<xref ref-type="bibr" rid="cit39">39</xref>][<xref ref-type="bibr" rid="cit40">40</xref>].</p><p>2. TB является независимым предикторным фактором метастазирования в регионарные лимфатические узлы при опухолях рТ1 КРР.</p><p>При опухолях рТ1 КРР существует необходимость поиска надежных предиктивных факторов метастазирования в регионарные лимфатические узлы, определение которых позволит выявить потенциальных кандидатов для резекции толстой кишки. Многочисленные исследования, а также 4 метаанализа свидетельствуют о том, что TB является убедительным и независимым предиктивным фактором метастазирования в регионарные лимфатические узлы при опухолях рТ1 [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit6">6</xref>][<xref ref-type="bibr" rid="cit9">9</xref>][<xref ref-type="bibr" rid="cit18">18</xref>][<xref ref-type="bibr" rid="cit21">21</xref>][41–45]. Участники консенсуса настоятельно рекомендуют отражать TB в патоморфологическом заключении для опухолей рТ1 КРР наряду с другими предиктивными факторами метастазирования в регионарные лимфатические узлы такими, как низкая дифференцировка опухоли, лимфатическая и сосудистая инвазии, глубина инвазии в подслизистый слой [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit21">21</xref>].</p><p>3. TB является независимым предиктором для II стадии КРР.</p><p>В большинстве случаев решение о назначении АХТ основывается на классификации TNM, которая является стандартом для стратификации пациентов на группы, определяющие прогноз. Пациентам, имеющим III стадию заболевания, предлагается проведение АХТ. В то же время пациенты, имеющие II стадию заболевания, не рассматриваются как кандидаты для адъювантного лечения, за исключением случаев перфорации опухоли, лимфатической и сосудистой инвазии, распространения опухоли на серозную оболочку (рТ4а), низкой дифференцировки микросателлитностабильной опухоли, неопределенных или положительных краев резекции и менее 12 выявленных лимфатических узлов. Однако выживаемость некоторых пациентов со II стадией КРР значительно хуже, чем у пациентов с III стадией заболевания, получающих АХТ [46–48].</p><p>В многочисленных исследованиях и метаанализе было доказано, что степень TB является предиктором возникновения рецидива заболевания и низкой выживаемости пациентов с II стадией КРР [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit27">27</xref>][<xref ref-type="bibr" rid="cit28">28</xref>][<xref ref-type="bibr" rid="cit31">31</xref>][<xref ref-type="bibr" rid="cit33">33</xref>][<xref ref-type="bibr" rid="cit49">49</xref>]. Исходя из этих данных, участники консенсуса рекомендуют, чтобы TB был включен в число факторов высокого риска для II стадии КРР.</p><p>4. TB должен учитываться вместе с другими патоморфологическими предиктивными факторами.</p><p>Стандартные критерии классификации AJCC по КРР 8-го пересмотра были дополнены прогностическими факторами, такими как степень опухолевой дифференцировки (G), гистологический подтип, сосудистая инвазия (V), периневральная инвазия (Pn) и статус краев резекции (R), которые используются во многих клинических рекомендациях [46–48]. К молекулярно-генетическим маркерам относят: микросателлитную нестабильность (Microsatellite instability — MSI), статус генов K-RAS, N-RAS и B-RAF [<xref ref-type="bibr" rid="cit50">50</xref>]. Основываясь на имеющихся данных, участники консенсуса считают, что прогностическая ценность TB должна быть эквивалентна статусу G, V, Pn, поэтому рекомендуется учитывать TB вместе с другими патоморфологическими факторами при оценке риска прогрессирования КРР.</p><p>5. Для оценки TB используется окраска гематоксилином и эозином (ГЭ).</p><p>По данным большинства исследований, оценка TB выполнялась при помощи окраски микропрепаратов ГЭ. В нескольких исследованиях TB оценивали при помощи иммуногистохимии (ИГХ), в частности окраски с помощью антител к цитокератинам [<xref ref-type="bibr" rid="cit7">7</xref>][<xref ref-type="bibr" rid="cit8">8</xref>][<xref ref-type="bibr" rid="cit51">51</xref>]. По результатам 4 метаанализов установлено, что прогностическая значимость TB, оцениваемая при помощи окраски микропрепаратов ГЭ, значимо не отличается от ИГХ [<xref ref-type="bibr" rid="cit5">5</xref>][7–9]. Для оценки TB участники консенсуса рекомендуют выполнять окраску микропрепаратов ГЭ, так как большинство данных о предиктивной значимости TB получены при помощи окраски ГЭ (особенно для опухолей pT1). Кроме того, экономическая составляющая использования ГЭ позволяет выполнять оценку TB во всем мире. Также стоит отметить, что в некоторых случаях использование ИГХ является методом выбора по причине наличия перитуморозного воспалительного инфильтрата, что затрудняет оценку TB при помощи окраски ГЭ [<xref ref-type="bibr" rid="cit7">7</xref>][<xref ref-type="bibr" rid="cit52">52</xref>]. Иногда сложно дифференцировать TB от реактивных стромальных клеток или лимфоцитов. В таких случаях более оправданно использование ИГХ с антителами к цитокератину, что позволяет улучшить визуализацию TB, возможно окрашивание апоптотических телец и клеточных фрагментов, которые не следует учитывать как TB. Оценка TB с использованием окраски ГЭ и ИГХ возможна в сложных случаях, чтобы подтвердить, что выявленные опухолевые клетки или их кластеры действительно являются TB.</p><p>6. Наличие внутриопухолевого баддинга (Intratumoral budding — ITB) связано с метастазами в регионарные лимфатические узлы.</p><p>Внутриопухолевый баддинг характеризуется наличием отдельных клеток или их кластеров среди основного опухолевого массива, в отличие от обычного или так называемого перитуморального баддинга (Peritumoral budding — PTB), который оценивают в инвазивном фронте. Несмотря на то что термины ITB и PTB были введены в 2011 году, впервые феномен ITB был упомянут в исследовании, опубликованном в 1989 году, где была отражена его связь с наличием метастазов в лимфатических узлах [<xref ref-type="bibr" rid="cit12">12</xref>][<xref ref-type="bibr" rid="cit53">53</xref>]. Также в недавних исследованиях была отражена корреляция ITB с высоким уровнем PTB, метастазами в лимфатические узлы и степенью опухолевого регресса после неоадъювантного лечения [35–37]. Несмотря на то что ITB является перспективным предоперационным критерием для пациентов с КРР, в настоящий момент недостаточно доказательств для его рутинного использования в повседневной практике. Поэтому участники консенсуса рекомендуют оценивать ITB во всех будущих исследованиях для получения большего количества достоверных доказательств.</p><p>7. TB оценивается в одной горячей точке размером 0,785 мм², в инвазивном фронте опухоли.</p><p>Стандартизация имеет важное значение при внедрении любого прогностического фактора в клиническую практику. В настоящее время используется несколько различных систем оценки TB, о чем свидетельствуют результаты недавно опубликованных метаанализов и предварительное совещание членов консенсуса [<xref ref-type="bibr" rid="cit5">5</xref>][7–9][<xref ref-type="bibr" rid="cit21">21</xref>]. Несмотря на то что прогностическая значимость TB не зависит от используемой системы оценки, наличие единого международного стандарта имеет большое значение для будущих клинических исследований и ежедневной рутинной диагностики. Чтобы обеспечить стандартизацию оценки TB, участники консенсуса рекомендуют отражать в заключении площадь поля зрения, то есть мм², а не увеличение объектива (например, 20×), поскольку площадь поля зрения на одинаковом увеличении отличается между разными микроскопами. Площадь стандартного поля зрения, выбранная членами консенсуса, составляет 0,785 мм², что соответствует полю зрения, используемого Ueno и др., и принята JSCCR (Japanese Society for Cancer of the Colon and Rectum) — 20-кратный объектив с диаметром окуляра 20 мм [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit54">54</xref>][<xref ref-type="bibr" rid="cit55">55</xref>]. Также была разработана система для корректировки и стандартизации оценки TB для разных типов микроскопов (табл. 2). Для оценки TB рекомендовано использовать метод «горячих полей».</p><p>В большинстве исследований проводили оценку TB в поле зрения с максимальной плотностью опухолевых баддов, так называемый метод «горячих полей». В то же время в других исследованиях для оценки TB использовали осмотр нескольких полей зрения, в частности методы «5 полей зрения» и «10 полей зрения» [<xref ref-type="bibr" rid="cit5">5</xref>][7–9][<xref ref-type="bibr" rid="cit21">21</xref>][<xref ref-type="bibr" rid="cit56">56</xref>]. Методы оценки нескольких полей зрения имеют преимущество в том, что способны отражать состояние всего инвазивного фронта [<xref ref-type="bibr" rid="cit4">4</xref>]. С другой стороны, оценка нескольких полей зрения может искажать окончательное среднее количество опухолевых баддов. Таким образом, метод «горячих полей» лучше отражает максимальную степень TB. Участники консенсуса рекомендуют использовать метод «горячих полей», так как он основан на результатах большинства существующих исследований и договоренности специалистов, использующих этот метод [<xref ref-type="bibr" rid="cit57">57</xref>][<xref ref-type="bibr" rid="cit58">58</xref>]. Для выбора поля зрения с максимальным количеством опухолевых баддов необходимо, чтобы до оценки TB методом «горячих полей» был выполнен осмотр 10 отдельных полей зрения (20-кратное увеличение) вдоль инвазивного фронта.</p><p>Существует еще одно направление для оценки степени TB при помощи цифрового анализа изображений. Подход основан на том, что изображение, полученное после сканирования гистологического стекла, анализируется модулем, где происходит распознавание и дифференцировка клеточных структур опухоли в инвазивном фронте, которые численно отличаются от истинных опухолевых баддов. После этого модуль определяет размеры оставшихся опухолевых клеток и кластеров, считает их количество на площади, определенной консенсусом, равной 0,785 мм².</p><p>9. Cледует использовать 3-уровневую систему градации TB, чтобы упростить стратификацию риска прогрессирования КРР.</p><p>Для определения риска прогрессирования, основанного на оценке TB, в большинстве исследований использовались числовые интервалы, отражающие количество опухолевых баддов (2-уровневые и 3-уровневые системы). В то же время в нескольких исследованиях градация не выполнялась, а для прогнозирования риска развития рецидива использовали абсолютное число опухолевых баддов [<xref ref-type="bibr" rid="cit5">5</xref>][7–9][<xref ref-type="bibr" rid="cit21">21</xref>][<xref ref-type="bibr" rid="cit59">59</xref>]. Участники консенсуса рекомендуют использовать 3-уровневую систему, используемую JSCCR (табл. 3) [<xref ref-type="bibr" rid="cit60">60</xref>].</p><table-wrap id="table-3"><caption><p>Таблица 3. уровневая система градации Tumor budding</p><p>Table 3. 3-level Tumor budding grading system</p></caption><table><tbody><tr><td>Количество опухолевых баддов</td><td>Уровень Tumor budding</td><td>Степень Tumor budding</td></tr><tr><td>0–4</td><td>Низкий</td><td>Bd 1</td></tr><tr><td>5–9</td><td>Средний</td><td>Bd 2</td></tr><tr><td>10 и более</td><td>Высокий</td><td>Bd 3</td></tr></tbody></table></table-wrap><p>Эта система позволяет стратифицировать пациентов с ранними стадиями, а также у пациентов сo II стадией КРР. При ранних стадиях Bd 2 и Bd 3 связаны с увеличением риска метастазирования в регионарные лимфатические узлы [<xref ref-type="bibr" rid="cit20">20</xref>][<xref ref-type="bibr" rid="cit41">41</xref>][<xref ref-type="bibr" rid="cit54">54</xref>][61–66]. При II стадии КРР Bd 3 ассоциируется с повышенным риском рецидива и смертности [<xref ref-type="bibr" rid="cit9">9</xref>][<xref ref-type="bibr" rid="cit18">18</xref>][<xref ref-type="bibr" rid="cit21">21</xref>][<xref ref-type="bibr" rid="cit42">42</xref>][67–71].</p><p>Также участники консенсуса рекомендуют указывать абсолютное количество опухолевых баддов после степени TB (пример, Bd 3 (счет 15)). Это позволяет избежать потери информации, которая может возникнуть при использовании числовых интервалов. Например, Bd 2 (счет 9) может вполне соответствовать Bd 3 (счет 10). Как указано в пункте 4, важно, чтобы TB оценивался вместе с другими прогностическими патоморфологическими факторами;</p><p>10. TB должен быть включен в клинические рекомендации, а также в протокол патоморфологического исследования КРР.</p><p>TB — это хорошо зарекомендовавший себя независимый прогностический фактор КРР, способный повлиять на принятие клинического решения (см. пункты 2 и 3 [<xref ref-type="bibr" rid="cit5">5</xref>][7–9][<xref ref-type="bibr" rid="cit21">21</xref>][<xref ref-type="bibr" rid="cit59">59</xref>]. Стандартизованный, международный метод оценки TB обеспечивает основу для формирования патоморфологических заключений, включающих данные о его степени, в повседневной практике. Поэтому члены консенсуса рекомендуют включить TB в клинические рекомендации и протоколы патоморфологического исследования КРР.</p><p>11. TB и степень опухолевой дифференцировки — это разные патоморфологические прогностические факторы.</p><p>TB определяется как одиночные опухолевые клетки или опухолевый кластер, состоящий не более чем из 4 клеток (см. пункт 1), тогда как степень опухолевой дифференцировки характеризуется солидным компонентом опухоли. При многофакторном анализе прогностический эффект TB не зависит от степени опухолевой дифференцировки и структуры опухолевого роста [<xref ref-type="bibr" rid="cit4">4</xref>][<xref ref-type="bibr" rid="cit5">5</xref>][7–9][<xref ref-type="bibr" rid="cit21">21</xref>]. TB не снижает степень опухолевой дифференцировки, в отличие от низкодифференцированных кластеров. Таким образом, участники консенсуса считают, что TB отличается от степени опухолевой дифференцировки и имеет прогностическую ценность, независящую от нее.</p></sec><sec><title>Дополнительные клинико-патологические рекомендации оценки TB</title><p>При некоторых гистологических подтипах КРР (например, муцинозный, перстневидно-клеточный, медуллярный и микропапиллярный) оценка TB должна выполняться с осторожностью. При муцинозном и перстневидно-клеточном подтипе опухолевые бадды, погруженные в муцин, не должны учитываться. При медуллярных опухолях диссоциация и отделение воспалительных клеток приводит к невозможности их отличия от истинных опухолевых баддов. При микропапиллярных опухолях важно следить за тем, чтобы не включать низкодифференцированные кластеры опухоли (см. пункт 1). Также стоит принимать во внимание, что фрагментация желез часто связана с обилием нейтрофилов, что, в свою очередь, создает сложность для объективной оценки опухолевых баддов. Кроме того, качество гистологического препарата накладывает ограничения на определение степени TB, в частности наличие ретракционных артефактов, вследствие дефектов проводки и фиксации материала. В случаях, когда точное количество опухолевых баддов не может быть определено, в патоморфологическом заключении можно отражать как Bd (не может быть оценен) с указанием причин отсутствия объективной оценки.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>TB является важным прогностическим фактором для пациентов с КРР. TB Bd 3 в малигнизированных полипах может свидетельствовать о наличии метастазов в регионарных лимфатических узлах.</p><p>Таким образом, резекция толстой кишки имеет место после радикального эндоскопического удаления полипов, у пациентов с двумя и более неблагоприятными прогностическими факторами. TB представляет собой независимый прогностический фактор для пациентов с КРР II стадии. Выявление пациентов со II стадией заболевания и высоким риском прогрессирования с использованием дополнительных патоморфологических факторов важно для принятия клинического решения о назначении АХТ. Оценка TB в биопсийном материале первичного КРР косвенно может указывать на наличие метастазов в регионарных лимфатических узлах и отдаленных органах.</p><p>Несмотря на убедительную доказательную базу о клинической значимости TB при КРР, его оценка в повседневной рутинной диагностике не выполнялась, основным препятствием являлось отсутствие стандартных критериев оценки. В 2016 году был инициирован консенсус ITBCC (International Tumor Budding Consensus Conference), в ходе которого была достигнута договоренность о стандартизованной системе оценки TB. Сегодня TB включен в качестве дополнительного прогностического фактора в 8-м издании классификации AJCC, а также в ряде национальных рекомендаций Европы и Азии.</p><p>С клинической точки зрения становится ясно, что необходима валидация стандартов, разработанных на ITBCC, для оценки TB в ежедневной диагностической практике.
